Trip Consent

I agree to my child receiving emergency dental, medical or surgical treatment including blood transfusion and anaesthetic, as considered necessary by the medical authorities present. I understand that reasonable efforts will be made to contact the parents before administering treatment.

In the event that any emergency medical or other costs are incurred for my child, I will immediately reimburse Academic Families and process any insurance claim myself.
