If a school asks you to take a student from school due to mental health issues - do not agree instantly to remove a student or arrange a host for them until you have completed the form.

1. Insist that you talk to -

a) The most senior member of boarding staff, the Head of Boarding or DSL to get the background

i. Establish why the school think they need to be removed. Record details below

ii. How long they expect the removal to be needed for?

iii. Have the parents been informed of the removal?

b) The senior school nurse for medical information

i. Check if they have been assessed by medical professional. Record details below

c) The academic year head to understand any work expectations.

2. If possible arrange a meeting with all the above to discuss in person. Record details of the meeting below

i. Follow up to ensure that you receive all the necessary work information from relevant subject staff. Record details below if meeting has taken place.
